Create Caregiver Survey

Each partner organization that you partner with on the child-friendly map will do two things: They will facilitate Activity 1 and 2 with their youth groups AND they will send a map survey to their parent/caregiver community. We use the language of caregiver intentionally (rather than parents) because we see the term as more inclusive. Not all young people identify their caregivers as parents. What is the CFC Map Survey?
In the CFC map survey, caregivers are asked to engage with their children on what their favorite places to visit are. Caregivers are also asked to reflect on their favorite child-friendly city places from their perspective as caregivers. While their may be some replication on favorite places, this survey serves two additional purposes:
1) It offers a point of connection for children/youth and their caregiver/s and
2) It may generate different ideas or information to include on the eventual map that young people didn’t name on their own (during activity 2).
The CFC map survey is a Google Form and, as such, will compile all the data for you that caregivers deem most important for the creation of the map. We will be sharing more details about data collection in Stage 3.

- Again, go to the Your Templates folder on Google Drive and ensure that you have saved a copy of all of the documents within to your own Google Drive folder.
- Once you have, open the document entitled CFC Map Survey ENGLISH. We have also provided this survey in Spanish, at CFC Map Survey SPANISH.
- NOTE: When you open the links above they will open to the survey as a participant would see it. To edit the survey, open the copy you have made in your own folder.
